خلاصه مقاله: Background: Since the beginning of the corona virus 2019 (COVID-19) outbreak in Iran in 2020 major respiratory, neurologic, and gastrointestinal complications accompanied the virus. Some patients developed central nervous system symptoms, such as decline of level of consciousness, ataxia, dizziness, and peripheral nervous system and musculoskeletal manifestations. Methods: Data were collected from patients admitted by the infectious disease department of Imam Ali Hospital in Karaj province, Iran with symptoms of pneumonia, stroke, and SARS-CoV-2. Results: Among all patients admitted to the intensive care unit (ICU) with severe respiratory symptoms, we managed one patient with onset of respiratory distress followed by neurologic symptoms related to stroke. This patient presented with cough, fever, myalgia, and severe respiratory distress. Shortly after the onset of these symptoms, the study case demonstrated an altered level of consciousness and movement impairment. This patient’s brain CT scan identified specific findings of a hypodense lesion diagnostic of stroke. Additionally, in the brain MRI on this patient, a T2-weighted, hyperintensity in the parietal and temporal cortex was observed. Conclusion: The patient had no prior common stroke risk factors. Consequently, COVID-19 acts as an independent risk factor for ischemic stroke disease. Therefore, due to the outbreak of COVID-19 further studies are required to determine the definite effect of this virus as an independent infectious risk factor of ischemic stroke.
