Background and Aim

One of the most common primary malignancies in the brain is Glioblastoma (GBM), that has various treatment options such as surgery, radiotherapy, and temozolomide chemotherapy but the prognosis is still poor and the patients are faced with bad quality of life throughout the disease with median survival of 16 to 19 months. Therefore, Novel therapeutic approaches are required to improve outcome for patients with GBM




The therapeutic potential of vaccine immunotherapy for malignant glioma should not be underestimated. There are investigations regarding therapies which target tumor growth factor receptors and downstream pathways, angiogenesis, modulation of cancer stemlike cells, cell cycle regulation, oncolytic viruses, novel radiotherapy methods, and immunotherapy, including vaccines and immune checkpoints modulation like programmed cell death 1 and cytotoxic T-lymphocyte antigen 4. In addition to novel agents, there are developments in methods to get past the blood-brain barrier to facilitate central nervous system drug exposure. However, first generation tyrosine kinase inhibitors of EGFR and PDGFR, rapamycin mammalian target, Akt, and PI3K inhibitors have failed in clinical trials. The monoclonal antibodies ipilimumab and nivolumab inhibit CTLA-4 and PD-1, based on their efficacy observed in melanoma are now being tested in recurrent glioblastoma. Vaccines for glioblastoma are being investigated as another way to increase the activation of the patient’s immune system. Just as with vaccines against infections, tumor vaccines are used to prime the immune system against a specific antigen. Two types of vaccines are being explored: peptide based and cell based


A data eruption in the genetics, molecular biology, and immunology of glioblastoma has happened. Novel therapies may need cautious selection of patients depending on their mutations and protein expression of the tumor.
